Don’t take it so hard, Amanda. You can only design the form the best way you know how. You have no control over someone’s honesty in filling it out. As @seanfear said, have faith that most people will be honest.
Yes, the always disconcerting realization that information is not always factual. Father will know this, (Our Father already does). And many reasons for that. Fear, embarrassment, self-importance, manipulation. So many reasons.
