While I’m not interested in the actual existence of Adam and Eve, a strict analysis of the text does not require the conclusion that all future people were related to Adam and Eve. It says they were the first created, not the only people created directly by God. Reading further in the Cain and Abel story, there are already other cities when Cain goes into exile after killing his brother. Of course, these are not – nor were they intended to be – actual history (not that our current history books are any better at reporting the truth), so it doesn’t matter.
