Things this bad have happened. The Genesis spacecraft had an accelerometer installed backwards. The Mars Climate Orbiter burned up in the atmosphere of Mars due to a failed Imperial/metric units conversion. The Soviet probe Venera 14 was supposed to measure the compressibility of the soil of Venus, but a lens cap happened to fall in just the spot where its arm was going to take the measurement.

Relax. It’s all part of the Master Plan. We schedule another launch (or three) to correct the “problem”.

Job security, now you can send another one with a robotic arm to remove it…but hey why would a satellite even need a lens cap in the first place.

That would basically be the moon, then.

This happened to one of the Ranger lunar probes, Ranger 6, I believe. Ranger 7 was the first one to successfully complete its mission.

Ah… You do know what happened with the Hubble Space Telescope, right? Very Similar.
