I’ve seen a picture of an actual two story outhouse. Seems this guy had a boarding house, and he didn’t want his teenage daughters (wearing nightgowns) encountering boarders when they needed to use the facilities at night. So he built a second story onto the outhouse, accessible from the upstairs landing. Problem solved – as long as you got the upper seat!
