The claims process is phenomenally inefficient. Way back when Hillary Clinton was working on a (really quite fine) health insurance policy, then HHS Secretary Donna Shalala conceded that Clinton’s plan had one definite flaw: 600,000 people more or less would lose their jobs, those being the platoons of specialists in the offices of hospitals and clinics who juggled a bazillion different plans issued by dozens of companies.
