Comics usually mimic reality in all it’s stupidity, and the virus is the main topic right now. So…..masks and their affect on every day life, are one everyone’s mind.
I am guilty of this, and I know, after exposing my elbow, that using my hand makes more sense (for reason’s noted above). I’ve been flushing urinals with my elbow, then washing my hands for years. It became habit after years of gross public bathrooms without soap or towels, where I didn’t/couldn’t wash my hands.
Soap and water are ALWAYS more effective than hand sanitizer which tends to evaporate before it kills all the viruses. Many have been recalled due to methanol contamination. Nobody ever went blind from using soap and water.

Why contaminate your shirt sleeve with an elbow pump when you are going to rub the sanitizer on both hands anyway?

Silly, use the first squeeze to sanitize the pump if it bothers you. Doesn’t matter since you’re not touching anything after you get your squirt.
