Waiter: Would you care for freshly ground pepper on your salad?
Brutus: Yes, please!
Waiter: There's the grinder - knock yourself out! Call me when you're done with it!
I prefer to pack my own take-home (doggy bag); you never know what happens to the food if they take your plate back to the kitchen to do it. Once, after checking the box and noticing that some of the meal that I wanted to take home was missing, I went to the kitchen myself and told them that I would take what I wanted and not what the waiter assumed I wanted. Since then I always insist that the waiter bring the box to the table and let me pack it myself.
I believe attitude is the key here. I prefer to use the pepper mill and fill my take-home box myself, but the waiter/waitress had better show some class, respect, tact, etc if they expect a tip from me!
