Why would anyone hide the end behind/under the roll.

I used to have mine under, and it was a good thing when my cat was young and frisky. She would spin it as hard as she could, but the paper would stay wound. I never had a mess to clean up!

Didn’t the Yardbirds have a song whose chorus went:

I heard a report on radio once, a lot of women were lambasting a reporter for saying over was better than under; the women said, “kids always unwind it if it’s over.” I have kids, and over/under matters not to them unwinding it at all. They make a mess anyway.

Over is the only correct way to go.

Sideways is sometimes necessary to start those huge rolls you get from Costco.

The reason that over works best has nothing to do with technique, sanitization, or any of that other stuff…=It’s because the Mrs. will be happy!…(XD)

In Japan, over is the way to go simply because most of the TP here is not perforated. There is a metal or plastic flap that covers the roll and aids in tearing off the desired amount of TP.
