Before getting a check up, a dentist office insisted that I sign a form that claimed the Doc had told me about any side effects of “the procedure”. I had not even seen him yet so he didn’t know what, if anything, he was going to do so obviously hadn’t told me ANYTHING yet.
Ah the form that says that you fully agree that the doctor isn’t responsible for any misery you get because of his actions whatsoever, and also that you blame yourself in case you die and not your doctor.
Common sense and our local medical facility are mutually exclusive. Took a woman (neighbor) in who had a severe laceration crossing the palm of her hand and up and across her wrist. I had bleeding controlled with a pressure wrap. At the check-in window they unwrapped the hand (although I had advised against it). Blood squirted on the window and counter.
After they rewrapped it, we waited around an hour for them to get her into the ER. Three days later she had to go back – her two middle fingers wouldn’t bend – they had neglected to reconnect severed tendons.
Unless that’s an antique vase that Antiques Road Show appraised for $10,000, go find someone with a sledge hammer (and a somewhat delicate touch). Problem solved!

I cut the tip of my thumb off, had it all bandaged up and they expected me to fill out forms at the emergency clinic!

Is the patient a Marine? One (pejorative) synonym for a Marine is “Jarhead.”

Most places you can do the form verbally. They ask the questions and you answer them.
