This is one of the reasons for hospitals switching to electronic medical records-you couldn’t read half of the doctor’s or nurse’s notes (the primary reason was a government mandate for patient portable medical records). Of course now they use dictation software which is almost as bad-what the software heard isn’t what the doctor said and sometimes it is a real hoot to read what the program translates into print.
