It’s a face mask. The person’s nose is at the left, covered by the mask. Above the mask is the person’s eye, closed. The edge of the mask is a skull, aimed at the person’s ear. The strap of the mask goes around the back of the ear.
Looks like a home-made mask, or the sort that helps contain sneezes inside, but does little or nothing to protect the person wearing it. So perhaps a comment on the unavailability of reliable masks, and the false trust too many people (health care providers above all) are having to put in masks that are not fit for purpose.
