As far as I’m aware there aren’t many mammals that live longer than 40 years (or even much longer past their reproductive age,) it’s possible that they just don’t live long enough to have to deal with conditions we humans with our near century of life have to worry about.
The reason humans have bad backs is because we are not yet fully evolved to stand upright. We’re most of the way there, but not yet.
We also have lots of lung problems for the same reason. Not to be gruesome, but too many things end up draining into our lungs that horizontally-oriented animals don’t have to deal with.

Sometimes they fight and kill each other hitting with their necks. Google the giraffe with the zig-zag neck.
