I know about the missing-kidney urban legend, but I don’t see that a woman in a raincoat is an essential part of the myth. I’m happy for those who find the cartoon hilarious; I just don’t.
(Supersurfer: I haven’t thought of carbon tet in ages. We used to use it to wash off nuclear weapons components.)

Is the cartoon just surrealist silliness or is there some in-joke I’m missing?

It’s not carbon tet (which I used to use to clean my modelling paint brushes), but chloroform used to knock out the little buggers.
