Mid-1980s, the company I worked for set up direct deposit. My first paycheck didn’t get deposited (this was before you could check your account online, so I had no notification of this). Since this was in the days when I wrote checks for all my bills, I bounced a LOT of checks before I figured this out.
When contacted, the company’s bank (the 2nd largest in Chicago) said they couldn’t find my bank (the largest in Chicago, and I’d included a voided check) — banks located across the street from each other!
After some angst, my bounced check fees were covered.
A law school classmate of mine went to work for a firm that handled only banks’ and financial firms’ litigation. He had some horror stories to tell about his clients (since it all came out in litigation, it was public info). Banks do not hire the swiftest or most honest.

I went through this exact thing. Eventually it got straightened out but the credit union had to write a letter of apology and reverse the fees.
