The word “heart” also refers to the center and the core. In the phrase “with all my heart” refers to this center, this core. The sentence would more correctly be thought of as “I love you with all of the very core of my being.”
Her version would be more like saying “My love for you is all behind me”
As the Wizard said to the Tin Man, “Remember that the size of your heart is not judged by how much you love, but by how much you are loved by others.”
Looking at Aunty’s expression, I think she’s sincere. She really believes that the larger the body part you reference, the more love you are saying it contains. (Pretty much everybody’s ass is larger than their heart, by the way; your heart is approximately the size of your fist.) But despite her attempt to show just how big-hearted (um, big-assed?) she truly is, I doubt that her modification of this well-worn expression is going to catch on anytime soon.
