I’ve always assumed that either a: the horn grows in after birth or b: it is soft and flexible like a baby rhinoceros.
Incidental thought: Is there any other placental mammal species for which giving birth is as much agony as for the human? (Decades of nature documentaries makes me suspect the answer is no. The price we pay for big brains.)

This isn’t realistic because the horns on baby unicorns are proportionally smaller.
