This is usually the result of thinking too far ahead in the conversation, or about something else altogether.
A somewhat more disturbing situation is when I find myself talking more-or-less by rote, but it’s the WRONG rote, leaving me sometimes unsure what I had just been saying. It’s sort of like an autocomplete error on a smartphone. Again, it happens when I am simply not paying attention to what I am saying. We’ve all done the same thing while driving, where we are in one sense aware of the road and can react to changes, but our mind is wandering somewhere else and we are driving on autopilot, only to find ourselves surprised at where we are.
