I’ve stayed at some pretty marginal motels, but none quite THIS worrisome. Including the one whose doorway was so out of true that the ground squirrels ran in and out of the room freely.
lol the one I stayed at, the bar you hold onto in the shower fell off the wall when I touched it and the brick pillar in front of my room had been hit by something and was crooked and missing bricks lol
Reminds me of the place on Vancouver Island we stayed at. Bare cinder block walls. Cast iron frame bed. Stained mattress. I was looking for syringes under the sink.
Must be our run-down local “OD” motel, where we keep the societal dregs of humanity. Their business motto is, “if you think you’ve reached the bottom, you have!”
That motel may be better than the other ones in the area. The others may require you to get your own transportation to the emergency room and pay for it out of pocket.
Research and planning. Research and planning. It took me about 50 years to train my spouse to plan our trips. He just wanted to ‘go’ and ‘stop’ when he was tired of driving. Actually, I ended up planning our trips after the first couple of years [he now plans a few trips on his own]. It’s much easier now to find clean, safe places to stay when you can check them out online — although you do have ‘consider the source’ and double check reviews [there are trolls everywhere]. FYI, there are laws against posting false reviews (both positive and negative). In the US, 15 US Code § 45, the Federal Trade Commission (FTC) has the power to stop and penalize parties “using unfair or deceptive acts or practices in or affecting commerce.” And the FTC forbids the use of fake testimonials. I’m sure other countries have similar regulations. So if you get succored into a bad motel situation by apparently deceptive reviews, notify the appropriate trade commission. It won’t help your experience, but it might help some others.
I travel (or used to pre-pandemic) with a friend and she has an uncanny ability for finding “interesting” lodging. One that leaps to mind had rooms with NO way to control the temperature in the room.
I recall a cartoon from years ago where a series of rooms had signs reading “Do Not Disturb”, “Do Not Disturb”, “Do Not Disturb”, and “Disturbed”. Probably good to avoid that place as well.

If the motel host asks you “Da ya wants da room for two o’clock, three o’clock or four?”, try another motel with by-the-day or weekly rates.

He’s still not quite sure about this sign.
It could be a good sign (depending on the neighborhood?).
